Privacy Policy
Effective date: September 25, 2026
Alliance Veterinary Procedure Center (“AVPC,” “we,” “us,” or “our”) respects your privacy. This policy explains what information we collect through our website and online forms, how we use and share it, and how you can contact us with questions.
Information We Collect
Information you provide. When you contact us or submit an appointment request or referral, we may collect your name, mailing address, phone number, email address, referring veterinarian’s information, and information about your pet. This may include your pet’s name, age, species, breed, weight, medical history, care needs, and information in files you choose to upload, such as medical records, laboratory results, images, or radiographs.
Information collected through website use. When you visit our website, we and the providers that support the site may automatically receive technical information such as your IP address, browser and device type, pages viewed, referring website, and the date and time of your visit. Cookies and similar technologies may be used to make the website function, understand site traffic, and measure the performance of our outreach.
Online Forms and Jotform
Our online appointment and referral forms are provided through Jotform. When you submit one of these forms, Jotform receives and processes the information and files you provide on our behalf. Our team reviews the submission and enters relevant information into our practice management system to respond to your request, coordinate your pet’s care, and maintain your pet’s medical record. Form submissions may also remain in our Jotform account according to our account settings and retention practices.
If you authorize us through the form to share your pet’s information or medical records with your primary veterinarian, we may share them for care coordination. Jotform’s privacy practices are described in its Privacy Policy.

We may use information we collect to:
- Respond to inquiries and appointment or referral requests.
- Communicate with you about your pet’s care.
- Coordinate care with your pet’s primary veterinarian when you authorize us to do so.
- Review records and information relevant to your pet’s care.
- Create, update, and maintain practice records.

- Protect the security of our systems and respond to legal requirements.
How We Share Information
We may share information as needed with service providers that help us operate our website, receive and manage forms, maintain practice records, communicate with clients, or understand website use. These providers may process information on our behalf under their applicable terms and agreements.
We may also share information with your pet’s primary veterinarian when you authorize us to do so, or when disclosure is required by law or reasonably necessary to protect the rights, safety, or security of AVPC, our clients, or others.
We do not use information submitted through appointment or referral forms for unrelated purposes.
Cookies and Website Analytics
Our website may use cookies and similar technologies for site functionality and to understand how visitors use the site. You can adjust your browser settings to refuse or remove cookies. Some website features may not work as intended if you block cookies.
Some pages or features may rely on third-party services. Those services may collect information through their own cookies or technologies when you use them. Please review the relevant provider’s privacy policy for details.
Third-Party Websites
Our website may link to services operated by other organizations, such as online forms, a pet portal, maps, social media, or payment-plan providers. Those services are governed by their own privacy policies and terms. AVPC is not responsible for the privacy practices of websites or services we do not operate.
Data Retention and Security
We retain information for as long as reasonably necessary for the purposes described in this policy, including to coordinate and document care, maintain business records, meet legal obligations, and resolve disputes. Online form submissions may be retained separately in Jotform according to our account settings and retention practices. Deleting a submission from Jotform may not delete information already entered into our practice management system.
We use reasonable administrative, technical, and physical safeguards designed to protect the information we maintain. No method of transmitting or storing information can be guaranteed to be completely secure.
Your Choices and Requests
You may contact us to ask about, correct, or request deletion of information you have provided. We may need to retain some information for care, recordkeeping, legal, or security purposes. Depending on where you live, applicable law may provide additional rights regarding your personal information.
